iTELConnect's flat rate virtual PBX is available with a local phone number or toll free number. Because it is a flat rate service it is a little more expensive than a pay per minute service but not that much more.

iTEL-Connect Virtual PBX has been designed for the small to mid-sized office to provide quick and easy call routing and provide a more professional appearance for callers. The iTEL-Connect Virtual PBX includes a front-end Auto Attendant, a Dial-by-Name Directory plus one or more iTELConnect services.

VoiceNation also provides a flat rate PBX in 85% of the LATA's in the US. The catch is that the flat rate only applies for calls going straight to voicemail. If the call forwarding feature is used and the call is connected "live" they apply a minimal three cents per minute.
